Unexpected and unwanted outcomes, accidents and injury are the products of flawed system design. The challenge in incident investigation and root cause analysis (RCA) is design challenge: indentify systems design features that cause accidents and then develop effective re-design strategies.
This webinar describes how RCA drives comprehensive safety management strategies and the importance of learning from “near miss” events. It will review barriers and risks to effective RCA that are specific to health care, including system and process complexity. This presentation will provide related training resources to help health care staff implement effective programs of incident investigation and RCA in their organizations.
